Bringing someone else’s minor rider?

An appointed guardian or authorized adult must bring a notarized statement from the parents or legal guardian giving that adult permission to be responsible for the minor and sign releases. The authorized adult must remain on the premises while the minor is at ACP. Consider bringing two notarized originals because ACP may retain one with the waiver.
